Application form
This page enables you to download a copy of the housing and council tax benefit application form and any of its attachment that you may need.
Once printed complete the form and return it to the address in the contact table. If you wish to call in person the customer service reception is open Monday, Tuesday, Thursday and Friday 8.30am to 5pm, Wednesday 9.45am to 5pm.

Housing and council tax benefit application form - after printing the application form complete it and return it to the housing benefit office. If you do not have all of the proofs required do not wait. Return the form explaining that some proofs are still to be supplied. As soon as you have the missing proof send, or deliver it, to the housing benefit office. We cannot process your claim until you have given us all of the proof needed. Landlords rent proof form - this form can be printed and passed to your landlord if you cannot prove the rent you pay by supplying your tenancy agreement or rent card. Your landlord will need to complete the form and return it to the housing benefit office. File format Adobe Acrobat (pdf) - 25KB |

Employers certificate of gross earnings - this form can be printed and passed to your employer if you are employed but cannot prove how much you are paid by supplying the required number of wage slips.
