This page enables you to download a copy of the housing and council tax benefit application form and any of its attachment that you may need.

Once printed complete the form and return it to the address in the contact table. If you wish to call in person, the customer service reception is open Monday, Tuesday, Thursday and Friday 8.30am to 5pm, Wednesday 9.45am to 5pm.

If you don't have all of the proofs we need don't wait - return the form explaining that some proofs are still to be supplied. As soon as you have the missing proof send, or deliver it, to the housing benefit office. We can't process your claim until you have given us all of the proof needed.

If you can't prove the rent you pay by supplying your tenancy agreement or rent card, your landlord will need to complete the landlords rent proof form and return it to the housing benefit office.

If you are employed but can't prove how much you are paid by supplying the required number of wage slips, your employer will need to complete the Employers certificate of gross earnings form and return it to the housing benefit office.
