Choosing a service

Making a referral to a service is very simple process:

• decide which of the client group descriptions best describes the needs of the person requiring support. Services are grouped together by the primary client group they work with. Some services provide support to more than one client group and this is usually indicated in the service descriptions
• decide if the person requiring support needs an accommodation based or floating support service
• select the most appropriate service and contact the provider to make a referral
• if you are unable to find a service appropriate to your needs there may be other options available, listed under one primary client group but accepting referrals from other client groups. For further information please contact the Plymouth Supporting People Team
