Support plans

When a tenant first moves into sheltered housing, the sheltered housing officer will meet with them to discuss a support plan.

All sheltered housing providers have introduced these plans to help monitor that tenants are receiving the correct level of support. This is a confidential document.

This means that only people who have access to this information are the tenant and staff on a need to know basis. If a tenant would like to see any information that is kept on file about them, they should contact their sheltered housing officer.

Other service providers, for example social services, will only be contacted if a tenant agrees to us doing so. We will only share information with others where it is agreed that we can.

Why is a support plan needed?

It will help identify the support that is needed by the tenant. It also helps us to provide them with the most appropriate support to help them to stay independent and look after their health. The sheltered housing officer can assist in co-ordinating these services and linking with new ones.

What if circumstances change?

We review support plans every six months. If a tenant wants to discuss it sooner, or there are any major changes in circumstances, tenants are advised to notify the sheltered housing officer, who will seek to review it.

Any reviews will be at a time and place that is convenient to the tenant.
