Application form

To obtain a Plymcard please take your completed application form to any of the following locations:

• First Stop, Civic Centre
• Central library and all branch libraries
• Phone us on 01752 307001.

You'll need to take along proof of address (and proof of status if applying for a free card).

Alternatively, return your application form to us at The Plymcard Office, Community Services, City of Plymouth, Plymouth, PL1 2AA and we'll post the card to you.
