Serious case reviews
A serious case review is carried out by the Local Safeguarding Children Board when a serious incident or death of a child occurs.
Reviews are carried out to determine what lessons, if any, can be learned by the agencies involved. The purpose of a serious case review is not to assign blame.
Agencies involved with the child in question compile reports about their actions and involvement and submit it to the Serious Case Review sub-group of the Local Safeguarding Children Board. The sub-group then commission an independent investigator to look into the reports and interview the staff involved to determine of there are lessons to be learned. A serious case review report is then compiled and discussed with the relevant managers and staff of the agencies involved.
