Lifestyle choices project
The 'Lifestyle Choices Project' is a unique project set up to help people with a physical or sensory disability. The project is run by Livability, a charity which comprised of two charities:
- John Grooms
- The Shaftsbury Society
Plymouth City Council adult social care service have commissioned Livability to deliver the Lifestyle Choices Project to adults living in Plymouth.
The project is free and aims to support disabled people to become more involved in social, physical or educational activities and get back into the community.
The project provides support to all areas with post codes beginning with PL. There is a simple referral process, where an individual can self refer or family member, care worker or a member of social services can make a referral on their behalf.
The process is as follows:
- A referral is made to Lifestyles.
- Initial contact will be made to the client by Lifestyles.
- A visit to the individual in a place of their choice to discuss possible activities is arranged.
- Research will be carried out by a support worker to find out about various chosen activities and see what is available in the local area.
- The support worker will arrange to meet the client again to go through what activities have been found, and if one of these is suitable, arrangements will be made to make a visit.
- Support is provided to the client to visit the activity for the next few sessions.
- Lifestyles will continue to support the client on a regular basis.
- Lifestyles will then encourage the individual to carry out the activity as independently as possible.
- When the client feels they have enough confidence, they can continue to access the activity independently.
