Home care reablement service
If you have lost some of your independence, perhaps due to a recent illness, hospital stay or changes in your care arrangements, this service may be able to help you get back up to speed.
The service is short term and free for people over 18 who live in Plymouth and whose safety or independence are at risk.
You might need extra support because you:
- want to regain skills and confidence to live independently following an illness
- are returning home after a period in hospital or residential care
- have developed physical disabilities
- are older and need support to re-gain independence in your own home
