16+ service

The 16+ service provides services for young people looked after by the local authority and leaving care to make sure that they are properly supported, both practically and emotional.

The team work with young people aged over 16, living in and leaving care, and continue to support them until they are 21, or 24 if they are in education.

What we do

Every young person over the age of 16 who leaves care will have a personal adviser. This person will work with them to ensure that they have an assessment and a pathway plan. This plan is about where they want to get to in their life and how we will support them to get there.
