Single point of contact

The Single Point of Contact (SPOC) is the main route of access to multi agency services within Plymouth’s Children’s Integrated Disability Service (ChIDS)

SPOC is part of the Common Assessment Framework (CAF) process and links services for children and young people aged 0-18 years who have a disability and/or multi disciplinary needs requiring support from specialist teams within ChIDS and the Child Development Centre (CDC).

If appropriate a Team Around the Child (TAC) approach will be established and a lead professional identified. The aim for each family is that universal, targeted and specialist services will work together in order to support the individual needs of every child and family.

The SPOC aims to:

  • Coordinate services with families at the heart of decision making
  • Reduce the need for families to have to keep repeating themselves
  • Recognise the unique needs of every child and family
  • Offer earlier interventions for children with disabilities to prevent problems escalating and to increase the likelihood of a positive outcome.

SPOC will link with a range of teams including:

  • EarlyYears Inclusion Service
  • Advisory Team for Sensory Support
  • Communication Interaction Team (CIT) including EarlyYears (CITEY)
  • Children’s Disability Social Care Team including the Transitions Team
  • AHELP (Aiming High  Enhanced Lead Professional) Team
  • Children’s Disability Occupational Therapy Team
  • Services within the Child Development Centre
  • Special Services
  • Learning Disability Nurse
