Preparing to leave care

You will have a Personal Advisor from the age of 16 until you are 21, but can choose to keep seeing them or have support when you need it until your 25th birthday. Your Personal Advisor will become your main point of support and contact from 18. Until then, you will continue to have a social worker. Your Personal Advisor will offer advice, support and guidance after you leave care to ensure you reach the positive destinations in life that you want to. This might include:

  • Education, training and work opportunities
  • Making friends and support with relationships
  • Supporting you to find somewhere good to live and become independent
  • Support with money or benefits
  • Support with your health and well-being

Your Pathway Plan

You are entitled to a Needs Assessment and Pathway Plan that sets out the goals you want to achieve in life and how we can help you achieve them together. Once you turn 16 your Pathway Plan will replace your Care Plan. We will review your Pathway Plan with you at least every 6 months. With your agreement, we will seek the views of other people and services that are important in your life. This is to ensure we all work together to provide you with clear and consistent advice and support.

Together with your PA, they will also consider what extra support you may need.
